Back in my marketing days i used to use 4imprint (amongst many others) who were pretty good.
If you google for promotional merchandise you’ll get loads of companies that do the same.

Great lil program, affiliate program actually.. UnholyKiz, another CAP member here draws some designs for me, and others I do with just text.. I have only given them away thus far, but will be offering them for sale at ladies of holdem here soon
