888 locked my affiliate account for “lack of activity” soon after the recent spam stuff surfaced. I’m clearly small fry. Some programs have stuck with me even when I’ve not provided much traffic and I’ll repay that one day.

Re. locked accounts – programs that do that bite themselves. You never know who will be the next big affiliate. Especially large directories often suddenly take off and individual places do well all of a sudden, and small sites can do that too. Not a sound policy.
