META Description and TITLE tags are two of the most important elements on every page and should be very relevant to the content and descriptive to visitors. Google uses both in it’s search results so they have to a) be written in such a way as to make Google see it as worthy of a decent position and b) to make a visitor want to click it based on what they read.
And yes they should be unique to every page in both instances.
