I believe the signup page that you’re talking about is the in-house UB affiliate program signup page. If you’re having any difficulty with signup there, you can email [email protected] and they should be able to help. 
There are two parts to the UltimateBet affiliate program – one is run through Income Access, and the other managed in-house at UB – and they have the same commission structure.
