I would strongly recommend VIP Profits and their VIP.com casino — meets those criteria for me.
You will find a regularly updated list with all programs that will continue to accept US players at

I’ve tried to use their ‘forgot password’ link and it takes me to a 404 error page at https://www.vipprofits.com/affiliates/forgotpassword1.asp
And a message gives a fake telephone number: “You have exceeded the allowed number of tries to change a password. Please try again later or call 1-800-xxx-xxxx. “
Hardly reassuring.
