I will take the necessary steps to prevent this from happening again and contact the affiliate.
Of course I can’t disable an affiliate account until the person has been given a chance to explain their actions and make ammends, but you have my word that it will be dealt with accordingly.

This matter has been resolved. I’ve concluded this affiliates unique affiliate code was incorporated by a third party source through deceit. This affiliate was not a known willing participant in the affair.
